In case you are one of the following, then you need to get the report from the Medical Council of Canada and the Date designated is April 17, 2013:

In case you are a pharmacist, then the report must be done from Pharmacy Examining Board of Canada (professional body for Pharmacists) and the designated date is January 6, 2014.
